The Change of Smoking Rates before and after Hospital Admission Following Diagnosis of Ischemic Stroke

Abstract
Background:Smoking is an established risk factor for stroke, and previous studies showed that the risk of stroke declines after smoking cessation. But the prevalence of smoking cessation after stroke has rarely been studied in Korea. The purpose of this study was to examine the smoking cessation rate and factors related smoking cessation after stroke.

Methods:All patients admitted to the neurology department with first ischemic stroke from August 1, 2006, to December 31, 2008, were identified. All of them were current smokers at admission to hospital. We estimated stroke risk factors and characteristics related to smoking of patients who kept smoking and quit smoking. Seventy two patients who participated in the telephone interview were included in our study.

Results: After mean follow-up of 18.9 months, those who kept smoking after stroke attack were 43/72(59.7%), and 29/72(40.3%) quitsmoking. The number of cigarettes per day before stroke attack was significantly greater among those who kept smoking than those who quit smoking (P=0.048). Amount of smoking after stroke was reduced compared to before stroke, even among those who kept smoking. However, duration of smoking, risk factors for stroke, and drinking status were not significantly different between continuous smokers and quitters. Most patients tried smoking cessation on their own and unassisted.

Conclusion:After stroke attack, most patient attempted to quit smoking but only less than half succeeded in smoking cessation. Active intervention with various strategies should be given to help long-term smoking cessation for stroke patients.
